I had two Hysertic Sign sent to thegrave during the same turn, only ONE of them should be activated, but it didn't occur at all. I never activated the eff to add a Elegant Egotist during the same turn.

Please provide a replay of that happening. Also keep in mind, Sign only triggers when sent from Hand or Field. If it was send directly from the Deck, it won't activate.
